Step 1: From your Forms dropdown choose, 'Add a Form'.





Step 2: Add a title to your reference (the customer will not see this). Add the form title (the customer will see)

Add Button text, email delivery options along with Captcha. Then Save Changes.



Step 3: You can now add a field to your form. Select add New Field



Step 4: You have several field choces to choose from



Step 5: Enter information. Then 'Save Changes



Step 6: You can now continue to add new fields.



Step 7: Go to Style and 'Overall Layout'. Select Forms, then Insert Module.



Step 8: Find your Form. Then 'Click Save'



Step 9: Click Publish.



Step 10: You have created your Form.
